Client Success Story: Regaining Independence at Home – Mr H’s Journey in Devizes

How tailored care made recovery possible

At Right at Home Chippenham, we understand how life can change in an instant, and how the right support can help someone reclaim their independence. Mr H, one of the clients we proudly support, experienced this first-hand after a fall turned his world upside down.

If you’re considering homecare for a loved one after a hospital stay or time in a care home, our Hospital-to-Home service – like the one Mr H’s story received – may offer the reassurance you need.

How Has Our Home Care Made a Difference?

Before his fall, Mr H was a 78-year old man, living independently in Chippenham with no care support. One day, while taking out the bin, he lost his footing and was admitted to hospital. From there, he moved into a care home to continue his recovery. But Mr H had one goal in mind: to return home.

That’s where we came in.

Our CareGivers began by visiting four times a day, supporting Mr H with everything from getting out of bed and preparing breakfast, to helping with personal care and exercises prescribed by his physiotherapist. Over time, we established a routine that supported his recovery, and that he could feel comfortable and confident with.

Each visit focused on both practical help and meaningful connection:

  • Morning visits included support with personal care, a cup of tea, and friendly conversation.
  •  Midday check-ins involved help with continence care, a home-cooked lunch, and gentle exercise.
  • Afternoon visits gave Mr H a chance to strengthen his mobility and enjoy another hot meal.
  • Evening support helped him feel safe and settled before bed.

Adapting Care as Confidence Grew

As Mr H began to regain strength, he gradually reduced his care to just two visits a day. This shift marked a major milestone in his recovery, not only was he stronger physically, but his confidence in managing day-to-day life had grown too.

Our team was there every step of the way, offering encouragement and consistency to help him feel in control of his own routine again. That balance between support and independence is at the heart of what we do.

Hearing from Mr H and His Family

“I’m incredibly grateful for the care I’ve received from Right at Home Chippenham. Their support made my recovery so much easier, and I always look forward to their visits. The CareGivers are not only helpful but also kind and caring — they’ve truly become part of my day-to-day life.” – Mr H

“My dad has come such a long way since coming home, and I believe the Right at Home team played a huge role in that. They’ve been fantastic at encouraging him while making sure he feels comfortable and supported every step of the way.” – Mr H’s daughter

Our CareGiver added:

“I’ve really enjoyed supporting Mr H. It’s been amazing to see his progress, and our chats over breakfast are always a highlight of my day!”

From Recovery to Routine - One Step at a Time

Mr H’s story is one of resilience and trust — trust in himself, and in our team. With the right care in place, he’s not only recovered from his fall but has found a renewed sense of independence at home.

It’s a journey we’re honoured to be part of.
