Hospital to Home

Our Hospital to Home service is a tailored service especially for people returning home after a hospital stay. It is a temporary package aimed at helping clients settle back at home safely and comfortably.

Who is it for?

Each Hospital to Home package is personalised to the individual client and their particular needs relating to the reason for their hospital stay, their level of ability, confidence and the type of support they need.

This service is for anyone of any age needing help with rehabilitation or adjusting to independent living after a hospital stay. Often our clients are elderly people preparing to leave hospital after a stroke, a fall or surgery. Sometimes, clients simply want the reassurance and companionship of having someone to help them as they adjust or regain their confidence.

We regularly work with clients leaving West Middlesex University Hospital and Kingston Hospital among others. We cover Twickenham, Richmond, Hampton, Sunbury and nearby areas.

How Hospital to Home works

The aim of this service is for a seamless transition from hospital care to living at home:

• Our care manager carries out two assessments, working with the client: the first in hospital and the second at the client’s home.
• Prior to the client’s discharge from hospital, the care manager will develop a tailored care package to be discussed and agreed with the client and their loved ones if appropriate.
• From discharge, Right at Home Twickenham and Richmond will provide care in line with the care package, adapting as the client’s needs change.

The minimum time for our care is one hour but flexibility is at the core of Hospital to Home and each client’s need will be different.

Additional care services include:

• Understanding medical equipment, such as catheter care
• Linking with district nurses and occupational therapists
• Providing family members with support such as moving and handling, continence information and dementia support

What happens afterwards?

Hospital to Home is a rehabilitation package designed to be temporary. The aim is to help clients get back to normal, living independently and as actively as possible.
