Introducing our Nurse-Led Care Model

Here at Right at Home, we are proud to announce our new approach to our healthcare services: a nurse-led care model. This means we are placing highly trained nurses at the forefront of patient care. Enabling us to provide a higher level of care through their clinical expertise. We hope that this care model allows us to take a significant step forward in the quality and proficiency of the care we provide here at Right at Home.

We are committed to continuing to improve our health care services. With the addition of nurse-led care, we hope our patients can benefit from their higher level of clinical knowledge which will enable them to provide a better service for all. This model is proven to improve outcomes and enhance patient satisfaction. Above all, our nurse-led approach represents the next step in our ongoing promise to deliver personalised, high-quality care. Care that is tailored to your needs and backed by the clinical expertise to support you at every stage of your health journey.

With our service expanding we would like to highlight the new clinical services we now offer to our patients. Our commitment to improvement has allowed us to create a care model which is better equipped to manage complex health needs, while improving overall comfort, safety and care, here is a breakdown of the new services we offer:

Catheter Care

Our nurses provide thorough catheter care supporting long-term health whilst focusing on maintaining comfort and reducing any risks. This routine involves regular catheter changes whilst promoting patient knowledge around catheter hygiene and maintenance. Our nurses and care providers are trained to monitor any signs of infection and blockages and ensure smooth day to day management.

Stoma Care

Living with a stoma bag can present daily challenges, both physically and emotionally but our team is here to provide expert support every step of the way. We are dedicated to daily care that focuses on maintaining healthy skin around the stoma site, ensuring secure and comfortable appliance management whilst addressing any concerns such as skin irritations. Throughout all our services, we prioritise patient comfort and dignity in managing stoma care.

PEG Feeding

For patients who require PEG (percutaneous endoscopic gastrostomy) feeding, our nurses provide hands-on support to ensure safe and effective nutritional care. This includes maintaining a consistent feeding schedule, PEG tube maintenance and cleaning whilst checking for any signs of infection. We also track each patient’s nutritional intake to ensure they are receiving the appropriate nourishment to support their overall health and wellbeing.

Diabetes Care

For our patients affected by diabetes we offer personalised care plans. By working closely with patients and their families we aim to help service users to take control of their diabetes with confidence and independence. As a service we offer support with routine blood glucose monitoring, insulin administration and dietary guidance to enable service users to manage their diabetes with ease and support.

Seizure Management

We believe seizure management requires proactive care. Our team creates personalised management plans for service users with the collaboration with their families and healthcare providers to ensure their safety under our care. Whether this be medication monitoring, emergency responses or monitoring triggers or changes in seizure patterns. Our nurses ensure care is customized for the individual needs of each patient.

Wound Management

Our nurses ensure quality wound care services for a wide range of conditions. Our nurses develop individualised treatments that encourage quicker healing, reduce the chance of infection and improve each patient's general comfort and quality of life while adhering to professional guidelines. 

Introducing our nurse-led care model allows us to provide all of our clients with a vastly improved healthcare service. We’ve seen continued improvements on response times and proactive health monitoring which enables us to identify and address any health concerns early on. Which in turn, reduces the need for hospital visits, easing the burden on families and allowing peace of mind for service users as they are guaranteed high quality care. In addition to this, our nurse-led care model extends beyond direct clinical care. It has enabled us to provide training and support for our caregivers. They are now trained and monitored to be better equipped to manage and support complex needs of clients. This additional layer of expertise enables for a safer and consistent care model for all.
