Meet Jon
Jon is 85 years old, and he lives in a Central London apartment with his wife, Sarah. They have built a long life together, and although Jon's health now asks more of them both, that life is still very much their own.
Their two children and their grandchildren live in the USA. It is a long way from Central London, so it is a connection Jon holds close.
Jon spent his working life as a professional and retired some years ago. He still takes real pleasure in an ordinary day. He reads books and keeps up with the news, and when the weather is fine he likes to get outdoors for a walk. He also treasures his wristwatch, which he wears every day. It is a small thing, and it means a great deal to him.
Jon lives with a number of long-term health conditions. Because he has a history of skin cancer and recurrent folliculitis, his skin needs daily attention. He also has chronic kidney disease and prediabetes, along with a long-term urinary catheter and a history of recurrent cystitis. His mobility has reduced over the years, so he can be unsteady on his feet, and he sometimes experiences episodes of confusion that can point to an infection.
These conditions ask a lot of Jon. He meets them with quiet patience, and Sarah meets them alongside him.
When Jon and Sarah first came to us, they wanted one thing above all else. They wanted Jon to stay well in the home they share. His desired outcomes were to:
- Move around his home safely and avoid falls.
- Keep his skin healthy and comfortable.
- Stay free from infection, with support at the first sign of a problem.
- Hold on to his daily routines and his independence.
- Stay involved in decisions about his own care.
These outcomes shaped our support from the very first visit.
How We Support Jon
Two Carers visit Jon every morning and every evening. The visits are practical, and they are personal too. Because Jon speaks English as his first language and prefers face-to-face conversation, our Carers take their lead from him and keep him involved throughout.
Staying safe on his feet
Our team assessed Jon's mobility and balance in his own home, where he uses a Zimmer frame for support. Our Carers help him to use it safely, and they watch for any sign of dizziness or unsteadiness during a visit. Walkways are kept clear, and support is offered ahead of every move. Independence matters to Jon, so he is encouraged to do as much as he comfortably can. If his balance changes, or if there is a near miss, it is recorded and reported straight away.
Looking after his skin
Jon's skin is checked every day, with close attention to bony areas, and our Carers support him with personal hygiene and skin care at every visit. Any redness or broken skin is escalated without delay. It is quiet, careful work that keeps Jon comfortable in his own body.
Catheter care and comfort
At each visit, our Carers check Jon's catheter tubing and the position of his day and night bags. They also look at the appearance of his urine for any change. Strict hand hygiene and PPE procedures are followed at all times, and Jon is encouraged to drink well throughout the day. Our team stays alert to the early signs of infection, such as a change in urine colour or a change in his usual presentation, and any concern is escalated promptly to the line manager. Throughout it all, his dignity comes first.
Everyday wellbeing
The small courtesies matter to Jon. Our Carers respect his routines and take his preferences seriously, and they offer support with the toilet at the right moments. A familiar face and a friendly word matter to him as much as the care itself. Sarah is kept involved wherever appropriate. Jon's blood pressure and temperature are checked at regular intervals, and the readings go into his care records.
The Difference Support Has Made
Jon is still at home, and he is still with Sarah. To them both, that matters more than anything else.
No falls have been recorded during his time with us. His skin has stayed in good condition, and his catheter care has been well managed. Because hydration and bowel care are monitored at every visit, his risk of infection and constipation has reduced.
Jon is treated with dignity and respect, and he remains involved in decisions about his care, so his preferences guide the support he receives. When his needs change, his care changes with them.
Any sign of confusion or shift in his mobility is taken seriously and acted on.
Sarah has peace of mind too. She knows the team well now, and she can feel that her husband is in safe hands. Communication with her and with Jon's healthcare professionals is clear and consistent, so everyone involved shares the same understanding of his needs.

Jon's care has taught our team a great deal, and we have shared that learning across the service. We have strengthened our awareness of skin integrity risks for Clients with reduced mobility, and we have reinforced how confusion can be an early warning of infection in older adults. We have reminded the whole team about prompt escalation and clear record keeping, and we have revisited best practice for Clients who live with long-term catheters.
Jon's experience shows the value of consistent and proactive homecare.
