Part-time, approximately 16–20 hours per week (working pattern to be agreed)
Job Purpose
The Care Coordinator is responsible for supporting the effective day-to-day coordination of care services, ensuring Clients receive safe, high-quality and reliable care while supporting Carers to deliver excellent service.
The role focuses on rota management, scheduling, communication and maintaining continuity of care. The Care Coordinator plays an important role in ensuring operational efficiency and excellent client satisfaction.
Key Responsibilities
Care Coordination & Scheduling
Create and maintain efficient care schedules and rotas
Allocate care visits safely and effectively based on Client needs and Carer availability
Respond promptly to rota changes, sickness, annual leave and emergency cover requirements
Minimise missed or late calls through proactive planning
Client & Carer Support
Act as a key point of contact for Clients, families and Carers
Communicate professionally and compassionately with all stakeholders
Support continuity of care by matching clients with suitable Carers
Help resolve day-to-day operational issues quickly and effectively
Administration & Compliance
Maintain accurate Client and Carer records
Update care systems and scheduling software as required
Ensure documentation is accurate, up to date and compliant
Support with audits and operational administration where required
Team Support
Work closely with the management team to support service delivery
Assist with onboarding and workforce planning where appropriate
Provide general office and operational support as needed
Emergency Cover
Support with emergency care-call cover only where absolutely necessary and as agreed
Ensure coordination responsibilities remain the primary focus of the role
Person Specification
Essential
Experience in a Care Coordinator, Scheduler, Administrator or similar role
or
Strong experience within domiciliary/home care
Excellent organisational skills
Strong communication and interpersonal skills
Ability to prioritise and manage competing demands
Good problem-solving skills
Confident using IT systems and scheduling software
Professional, calm and proactive approach
Desirable
Previous experience within domiciliary care
Knowledge of care planning or rostering systems
Understanding of CQC requirements and regulated care services
Experience working in a fast-paced office environment
