Recovering Safely After a Fall in North Leeds
When 99-year-old Mr M experienced a fall at home in the North Leeds area, his family were understandably concerned about his safety, mobility and recovery.

Creating a Tailored, Person-Centred Care Plan
Following a detailed care assessment, our team designed a personalised home-care plan to support Mr M’s short-term recovery goals and long-term wellbeing. His plan included:

The plan was reviewed regularly, ensuring our support evolved as Mr M regained his strength and confidence. As he progressed, visit frequency was reduced - from twice daily, to once daily, then eventually to occasional visits.
